   Alaska Advance Health Care Directive
You have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. This form includes a durable power of attorney for health care, lets you give specific instructions for any aspect of your health care to the extent allowed by law, lets you express an intention to make an anatomical gift following your death, lets you make decisions in advance about certain types of mental health treatment, and lets you designate a physician to have primary responsibility for your health care.

Advance directives are legal documents that are intended to clearly express a person's wishes about health treatment and care, during any time the person is mentally or physically unable to make decisions or communicate those wishes, and that are signed before he/she becomes mentally or physically incapacitated. This material, provided by the Disability Law Center of Alaska, explains durable powers of attorney, living wills, and declarations for mental health treatment.

A 28-page pamphlet that addresses common questions on Medicaid eligibility for nursing home care. This combined pamphlet also outlines CHOICE Programs (Community and Home Options to Institutional Care for Everyone) available to people who otherwise would have to be placed in a nursing home. Medicaid rules vary from state to state, so this pamphlet should not be regarded as a reliable guide for nursing homes or Medicaid questions outside Alaska. Warning: Congress changed several aspects of the law relating to Medicaid in February of 2006. This pamphlet will be updated soon to encompass these changes, but this has not yet been done.
