This brochure provides general information on Advance Health Care Directives, formerly known as Living Wills. An Advance Health Care Directive is a new document that became effective on July 1, 1999. This new document replaces two documents called a Living Will and a Durable Power of Attorney for Health Care.

Medicaid, Medicare, and Other Medical Assistance Options Medicaid is a needs-based program, administered by the Department of Human Services (DHS) Med-QUEST Division (MQD), that provides health insurance to individuals who are aged, blind, or disabled. Medicaid covers a wide range of services including: medical and behavioral health for adults and children, emergency dental for adults, full dental for children, and prescription drugs. Medicaid coverage is fee-for-service, which means you can go to any doctor who accepts Medicaid.
