? you rent from a private landlord and/or receive Section 8 assistance, AND
? your landlord has threatened to shut off your utilities OR has already shut off
your utilities, AND
? your landlord does not have a court order to evict you.
In addition to completing this packet, you will also need to complete a "Return of Service" form. You can pick up the Return of Service form from the District Court in your area.
