WHO WE ARE

FACES is a diverse, non-profit organization, originated in Harlem and established to assist persons affected by or living with HIV/AIDS. FACES employs a comprehensive, client-oriented, community-focused approach to provide education, outreach, legal advocacy, supportive housing and direct services to its clients. FACES services low-income and/or indigent persons who are predominantly diverse people of color.

HOW TO GET HELP
   To contact this group, CALL FOR AN APPOINTMENT. 212-283-9180, Monday through Wednesday from 9am to 5pm.
 
Location: 317 Lenox Avenue, 10th Floor, New York, NY 10027
Cross Streets: 126th Street and Lenox Avenue
Directions: Subway-2 or 3 to 125th Street; walk 1 block north to southwest corner of W. 126th Street Buses-M7, M60, M100, M101, M102 and Bx15 to 125th Street and Lenox Avenue; walk 1 block north to southwest corner of W. 126th Street NOTE: Need ID to be admitted to Bldg
Bring M11Q forms, doctor's forms or other medical statements as proof of HIV/AIDS status. Also bring Medicare, Medicaid, SSI, SSD, or any other entitlements cards that you use, as well as a picture identification.

Languages Spoken:  French, Spanish
Languages for which interpreters can be provided: As needed
Wheelchair Accessible: Y
Access for the deaf: American Sign Language Interpreter is available upon request.
Income Guidelines: 

  We use the federal income guidelines for low-income persons to help determine eligibility for our services.
 
Other Requirements: For help, you must be:
  HIV positive
  You must be a U.S. citizen or legal immigrant.
  This program does not serve prisoners

  Other- Referrals to other agencies are available.
