Bring the following documents to the appointment: names and addresses of the your doctors, therapists; names and addresses of hospitals and clinics where you are a patient; your hospital and clinic cards; medicines you are taking at this time (bring actual bottles); letters, notices and decisions from the Social Security Administration; a list of the your past jobs, and the dates when you had these jobs.
