This form is a mail-in application for Medicaid health care coverage for individuals with disabilities who are employed. There are two groups of coverage: Worker with a disability, and Worker with a Medically Improved Disability. It's intended to provide transitional health insurance for adults who: have incomes below 200 percent of the Federal Poverty Guidelines (approximately $17,000 for a single adult); who may have jobs that do not include health benefits; or who are in between jobs.
