Éste es un documento legal importante con el que hace una declaración sobre el tratamiento de la salud mental. Este documento le permite tomar anticipadamente decisiones sobre el tratamiento de la salud mental, y específicamente sobre tres tipos de tratamiento de la salud mental: los medicamentos psicoactivos, la terapia convulsiva y el tratamiento de emergencia. Las instrucciones que contiene esta declaración se seguirán solamente si un juez cree que usted está incapacitado para tomar decisiones sobre su tratamiento. De lo contrario, se considerará capaz de dar o negar consentimiento para los tratamientos.
