How to tell your doctors and agent what you want
Use legal forms to do this. You will find forms on this website that will let you:
Tell doctors what health care you need or want.
Ask someone else (your agent) to make health care choices for you if you cannot do it yourself.
Are these forms required?
No.
You do not have to use these forms.
Your doctors and insurance people cannot make you use them.
When should I fill these forms out?
If you want to use these forms, the best time to do it is before you get sick and have to go to the hospital or a nursing home.
If you do it ahead of time, you can relax, think clearly and make well thought-out plans about your future.
How do I use these forms?
If you use these forms, don't forget to sign them!
You must give a signed copy to your doctor.
You should also give a copy to your other health care providers.
Give a copy to your agent, the person you name to make your medical decisions.
You may also give a copy to a close relative, friend, lawyer or pastor.
If you are not able to sign these forms, someone else can sign them for you. The person signing for you must sign at your request, with you in the room, and with the witnesses or notary in the room when you sign
What if I change my mind?
If you change your mind after you have signed these forms you can take them back. This is called revoking them.
If you do not like these forms please see a lawyer.
These forms are legal documents. Do not try to write your own forms!
A lawyer can write a form that fits your needs. In order for these forms to work the way you want them to, they must be written the way the law says.

Using this website or sending an e-mail to Wyoming LawHelp does not mean that Wyoming LawHelp is your lawyer.
It is important to understand that if you email WLWP and put facts about your case in the email that WLWP will keep your information confidential. It is also important to understand that what you put in your email may not be confidential if you go to litigation on your legal problem. This means that the information you provide could be used against you in court. This is not a good thing! Think twice about what you write in an email! If you have any concerns at all, call our offices or talk to a lawyer!
For all these reasons, you may wish to use the telephone or talk in person when looking for a lawyer.
Deadlines
If you are handling your own case or do not yet have a lawyer, it is very important that you not miss deadlines! You may lose your legal rights if you do not take action right away.
This means getting a lawyer right away if that is what you want.
If you don't have a lawyer call the clerk of court to find out what deadlines apply to you.
