Information about using Self-Help Forms


Welcome! This online interview was developed by the Legal Aid Society of Hawaii for use by low-income persons representing themselves, however it is available free of charge to all persons. These forms are not for commercial use and charging for use in any way is prohibited.

Use this interactive interview if you would like to begin the court process of paternity or custody after VEP (voluntary establishment of paternity) and will file in Family Court of the First Circuit (Oahu). The interview will aid you in filling out these court forms: 1) Petition for Paternity and 2) Paternity Action Information. We suggest you read our publication Paternity: Establishing Custody, Visitation, and Support before completing this interview. 

 

What do I need to get started?

It is helpful to gather the following information before starting the self-help interview:

  • Your full legal name

  • Your mailing address 

  • The mother or father of your child's full legal name

  • The name of your attorney if you have one

  • FC-P Court number if you know it

  • Date, city, state/country (if foreign country) of your marriage

  • Full legal name of your child(ren), Gender, Date of Birth, and Birth Place

  • Your child(ren)'s address

  • Your child(ren)'s school 

  • Your child(ren)'s grade 

  • Where your child has lived in the last five (5) years and with whom he/she has resided 

  • Mother of the child(ren)'s information (if known): name, date of birth, last four of SSN, home phone, work or cell number, race or ethnicity, number of marriages, address, city/state of residence, employer, employer's address, job title, work schedule, time period of employment, gross monthly income, welfare income (if applicable)

  • If a Father is listed on your child(ren)'s birth certificate, provide his name, last four of SSN, date of birth, city/state of residence, employer, gross monthly income

  • Natural Father's Information (if known): name, date of birth last four of SSN, address, contact information, race or ethnicity, number of marriages, employer, employer's address, job title, work schedule, time period of employment, gross monthly income, welfare income (if applicable)

  • Legal Father's Information (if known): name, date of birth last four of SSN, address, contact information, race or ethnicity, number of marriages, employer, employer's address, job title, work schedule, time period of employment, gross monthly income, welfare income (if applicable)

  • Name and address of child(ren)'s Custodial Caretaker (if applicable) 

  • Child(ren)'s Medical or Dental Insurance Carrier 

  • Any other court cases involving any of the Defendant(s) and/or Child(ren)
